Population Health Project Manager

A company is looking for a Population Health Project Manager. Key Responsibilities Monitor and evaluate performance by tracking key performance indicators and identifying process improvement opportunities Lead quality improvement projects, ensuring alignment with timelines, goals, and contract requirements Collect, analyze, and interpret clinical and patient outcome data to inform population health management programs Required Qualifications Bachelor's degree in healthcare or related fields; MS and/or MPH preferred 3-5 years of experience in healthcare settings, with 1-3 years in a project management role preferred Strong understanding of Value-Based Care contracts and quality improvement concepts Proficiency in Microsoft Excel for data analysis and reporting Experience with EPIC/OCHIN and Microsoft Office Suite
