A company is looking for a Case Manager RN (Remote). Key Responsibilities Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs Develop proactive care plans to enhance members' short and long-term health outcomes Utilize clinical tools and data to conduct evaluations and address complex health and social indicators Required Qualifications Active and unrestricted RN licensure in South Carolina or compact licensure in state of residence 3+ years of clinical experience 2+ years of case management, discharge planning, or home healthcare coordination experience Associate's Degree in Nursing is required; Bachelor's Degree is preferred Certification as a Case Manager is preferred
