A company is looking for a Case Manager RN (Remote). Key Responsibilities Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs Develop proactive care plans to enhance members' overall wellness and outcomes Conduct assessments using various data sources to address complex health and social indicators impacting care planning Required Qualifications Active and unrestricted RN licensure in South Carolina or compact licensure in state of residence 3+ years of clinical experience 2+ years of case management, discharge planning, and/or home healthcare coordination experience Associate's Degree in Nursing (required); Bachelor's Degree (preferred) Must have active and unrestricted RN licensure in the state of South Carolina or compact licensure in state of residence
